According to the latest announcement, the Organizing Board of the Change Life - Season 5 Program will extend the application period until June 15, 2020, instead of the previous deadline of March 20, 2020.
Here under is the inscription information of the program:
* Target group: Male / Female citizens, aged 19 to 45 years old, living in Vietnam.
* Application requirements:
- Curriculum vitae certified by local authorities.
- Personal information: height, weight.
- Headshot picture with deformity from neck or above, photo size: 9cm x 12cm
(photos taken from left side, right side, from the top down, from the bottom up, photos of the whole face)
Note: video clip included in the applications introducing the candidates themselves (with duration of 45 - 60 seconds) is an advantage for the selection round.
- An application should specify how the facial/body malformation affects the candidate's life and what he/she wishes for life after surgery. Signature and stated consent of parents for the daughters and sons to participate in the program.
- A photo of valid passport in accordance with the law.
Invalid applications are applications missing any of the documents listed above.

2. Sent by post to the address: Department of Science and Education Programs, Vietnam Television, 43 Nguyen Chi Thanh, Ba Dinh, Hanoi.
